Medical History

This form will be used to better inform Nurse Christin in assisting your student at school.

Medication Administration

If you child has prescribed medication that must be taken during school hours or activities, parent/guardian AND prescribing doctor must complete this form.

Possession of Medication

If your child has prescribed medication for a medical emergency that is self-administered, i.e. allergy or asthma medication, parent/guardian and student must complete the Possession of Medication Form per Section 41,79-31, Mississippi Code 1972.
